The Baggies fell behind to a Blues team containing Michael Essien on the half-hour mark thanks to Jacob Mellis' goal in the Premier Reserve League (South) encounter.
But Lateef Elford-Alliyu finished off a strong individual run with a powerful shot two minutes before the break to level the scores at half-time.
Chris Wood put the visitors 2-1 up before they were pegged back by Milan Lalkovic. The game looked to be heading for a draw until substitute Nabi, who scored twice in the Under-18s' 3-0 win over Wolves on Saturday, fired home a left-foot volley with two minutes to go to take all the points.
"It was a good team performance," said Albion reserve/development coach David Oldfield.
"We did some good things in the first half but lacked a bit of quality on the ball.
"In the second half we played better and showed some good spirit against a good Chelsea team. It was good for the lads to play against some of their bigger names. We've had some younger lads in the reserves in recent weeks and we've made a few errors. But we worked really hard."
