But the game was marred by ugly post-match scenes at The Hawthorns.
Reading player Jobi McAnuff was sent off after the final whistle for violent conduct following a clash with Albion's Youssouf Mulumbu.
Then Reading's players were involved in a shoving match with Albion stewards as they attempted to celebrate with fans at the Smethwick End.
Robert Koren's double-strike looked set to hand Albion victory in their fifth-round replay – with help from Adam Federici's goalkeeping howler.
But a Gianni Zuiverloon clanger allowed Brian Howard to equalise for the visitors three minutes into stoppage time and Gylfi Sigurdsson blasted the winner five minutes into extra time.
Royals No 1 Federici gave Koren a huge helping hand when his allowed the midfielder's 47th-minute shot to squirm through his legs for the winner.
Koren had opened the scoring on six minutes but Jimmy Kebe equaliser just three minutes later.
There was a first-half blow for Baggies boss Roberto Di Matteo ,as key forward Roman Bednar limped off with what looked like a groin injury.
But huge cheers greeted the return of James Morrison as a second-half substitute after a 10-month injury lay-off.
The match began at break-neck speed and Albion had several decent chances before they eventually went ahead on six minutes.
A shot from the edge of the box by Roman Bednar was saved by keeper Adam Federici but Luke Moore raced onto the rebound and his cross hit Federici and bounced towards goal.
Ryan Bertrand cleared it off the line but the ball ran to Koren who rifled home off the underside of the crossbar.
But Reading were soon level as Kebe, who had scored after nine seconds of the original tie, took just nine minutes to find the net in the replay.
Jobi McAnuff got past Abdoulaye Meite too easily on the Reading left, and his cross was met by Gylfi Sigurdsson, whose shot was only parried by Scott Carson, with Kebe was on hand to turn in the rebound.
Matt Mills sent a free header wide from a Reading corner, Bednar missed a superb chance from a Koren cross and Moore fired just wide.
But Albion got their stroke of luck within two minutes of the second half as Koren collected a throw from Gianni Zuiverloon, turned towards goal and let fly with a low shot that flew straight at Federici but slipped through the keeper's legs and trickled in.
Reading could have been level within a minute as Sigurdsson unleashed a dipping shot from 25 yards, beating Carson but rattling the crossbar. McAnuff saw his follow-up spilled by Carson but was flagged offside in any case.
Carson rescued his side on 70 minutes with a smart low save from Sigurdsson after Simon Church had laid Kebe's cross into his path.
There was a scare for Albion moments later when a poor ball by substitute Chris Wood found Church but Carson raced out well to clear.
Reading grabbed their equaliser in the third minute of added time as Zuiverloon failed to deal with Federici's diagonal punt and Kebe nipped in, squaring for Howard who produced a calm finish past Carson.
Then, just five minutes into extra-time, the Royals went ahead for the first time.
A complete misjudgement by Meite presented the ball to Church who squared for Sigurdsson, who curled a brilliant 25-yard shot past Carson and into the top corner.
