Coroner calls for A5 junction changes after recent death crash

A coroner has urged road chiefs to make changes at a junction on the A5 after a young van driver died in a crash with a tractor.

Liam Clark, 21, died at the scene from head injuries as a result of the accident which happened near the junction with Streetway Way Road, in Shenstone near Lichfield on September 10 last year.

South Staffordshire Coroner Andrew Haigh has written to James Bailey, the commissioner for highways at Staffordshire County Council, calling for action to be taken.

The letter said: "Liam Clark died at the scene of a road traffic collision on the A5 at Shenstone on September 10, 2019.

"He had been driving a van and attempted to pass on the nearside an agricultural vehicle that was turning right.

"He did not realise that a boom protruding from the rear of the other vehicle would move into the gap he was seeking to use.

"During the course of the inquest the evidence revealed matters giving rise to concern. In my opinion there is risk that future deaths will occur unless action is taken.

"In the circumstances it is my statutory duty to report to you.

"The A5 is a trunk road used by all types of vehicles. There are numerous junctions. At the scene the speed limit is reduced to 50mph.

"I should be grateful is you could look at the scene to see if any improvements are warranted such as improved signage or road change lay out change.

"In my opinion action should be taken to prevent future deaths and I believe you have the power to take such action."

Mr Bailey has until April 4 to respond to the coroner's concerns giving details of the changes proposed or giving reasons if no action will be taken.

The report has also been sent to representatives of the two motorists, Staffordshire Police and the Chief Coroner Mark Lucraft.

Mr Clark, who was at the wheel of a white Citroen van, lived in Liverpool.

The van came to rest in a hedge during the incident which happened on the single carriageway between Wall Island and Weeford.

Mr Haigh recorded a conclusion that the driver died as a result of an accident. The tractor driver was unhurt.
