Hassan, who is originally from the Birmingham area, suffers from severe mental health issues, and police are advising anyone who sees him not to approach him directly but to call 999.
A West Midlands Police spokesman said: "Hassan is originally from Birmingham and officers believe he travelled back to the city since leaving the hospital and is currently staying in the area.
"He is described as Asian, with a shaved head and full beard. He was last seen wearing a white T-shirt, black Adidas sweatshirt, grey jogging bottoms, black sliders and purple socks with a grey coat.
"Hassan suffers from severe mental health issues and we are advising anyone who sees him not to approach him directly but to please call 999 immediately."
