The collision with the 9ft 3in (2.8m) bridge happened in Birmingham's Dale End, which has become notorious after similar incidents.
The 7.5-ton Isuzu commercial vehicle struck the underside of the busy overpass, which takes bus traffic in and out of the city centre, at about 10.10am on Thursday.
The male driver was shaken but unhurt and was on the phone to bosses to explain what had happened.
It is not the first time a trucker has been caught unawares by the bridge, which has a tapering underside and gets lower towards its centre point.
A lorry became wedged in the same spot in June, after another truck got trapped back in February.
