Wolverhampton-based Santos Care Ltd has been stripped of its registration by the Care Quality Commission (CQC), which effectively prohibits it from operating.
Inspectors found that some patients had received 'unsafe care'.
The company had previously been used by Staffordshire County Council to provide care for a small number of people, but the authority said it had since stopped using Santos's services.
Santos, which operated out of a shared office space in Berry Street, Wolverhampton, was placed under a tighter inspection regime after being declared 'inadequate' in October 2023.
But a further inspection in October last year found that, despite some improvements, care continued to fall short of what was acceptable. As a result, the commission stripped the company of its registration.
The inspection found three continued breaches of regulation related to the need for consent, safe care and treatment, and management oversight.

Andy Brand, the commission's deputy director of operations in the Midlands, said the decision was made as a last resort.
