Hudson Cole Perrins, who had a history of renal issues and congenital heart disease, was initially treated for constipation at Wolverhampton’s New Cross Hospital after his father Greg took him there on June 23.

The following day his mother took him back when his condition worsened, resulting in him being transferred to Birmingham Children’s Hospital, where he died four days later from a lack of oxygen to the brain, multi-organ failure and urosepsis.

At a hearing on Monday (December 9) Black Country Assistant Coroner Miss Kelly Dixon resumed the inquest stating that she found it was a "failing" for Hudson, of Sandbeds, Road, Short Heath, Willenhall, not to have been given a repeated heart rate check on his first visit in keeping with policy.

The inquest previously heard there was no report of further observations of his heart rate being taken by the staff.
