Kind hearted members and coaches of Withymoor Colts will take part in the game at Lye Town FC and line up against friends and family of Grace Holdnall.
The brave 11-year-old has recently been diagnosed with neuroblastoma and is currently receiving treatment at Birmingham Children's Hospital.
Her dad Dave will be part of the friends and family team on the day with her mother Laura and sister Georgia proudly shouting the Withymoor team on from the side lines.
Grace plays for Withymoor's girls team and special shirts are being made in club colours with her name on - the friends and family team will be playing in Red as she supports Liverpool.
A raffle will take place after the game with prizes including a 43-inch-television, hotel and spa break, a signed Aston Villa champions league football and match tickets and a £600 tattoo voucher - all money raised will go to Birmingham Children's Hospital where she is having treatment.
One of the organisers, Matt Webb said: "This has all come about at a fast pace, the diagnosis was only around six weeks ago but Grace is a massive football fan and so brave to go through this treatment."
"Lye Town have been great to let us have their ground, it is within travelling distance for most people so we are expecting a big crown and it should be a great occasion with all Grace's friends, family and team mates there."
Gates at the Stourbridge Road ground will open at 12.30pm on Sunday November 24 and admission is £5 adults and £1 for under 16s
