Stab death of Wolverhampton mother was preventable

The death of a woman who was stabbed by her husband in front of their 12-year-old son at their Wolverhampton home was preventable, authorities admitted today.

Iranian Kurdish woman Shadi Afraziyan, aged 36, died on December 29, 2011 after being stabbed 18 times in the chest and stomach at a flat in Newhampton Road West in Whitmore Reans.

Today, a review set up by West Midlands Police, Wolverhampton City Council and health bodies, said more could have been done to prevent her death. Her husband Behnam Shiraziyan, aged 39 at the time, was on medication for mental problems and was under the care of a health worker.

But authorities including the UK Border Agency had known since shortly after their arrival in the UK three years earlier that he was showing signs of depression.

Their 12-year-old son raised the alarm on December 29, 2011, when his father attacked his mother with a knife. She was pronounced dead at the scene.

In a report the Safer Wolverhampton Partnership admitted today that authorities could have predicted that Mrs Afraziyan or her son were at risk of serious injury or harm.

The report made 24 recommendations including insisting that agencies review their internal training.
