Inquiry over heroin death of young Daniel Jones
An?investigation will be launched into whether more could have been done to protect a 23-month-old boy who died from a heroin overdose, it was revealed today.
Social services, police and the Probation Service held an emergency meeting to discuss the death of tragic Daniel Jones and today announced a serious case review would be held into the tragedy.
The youngster was found dead at his home in Windsor Avenue, in Penn, on May 29 this year. His death was initially treated as unexplained but forensic tests have since revealed he died as a result of a heroin overdose.
His parents Simon Jones and Emma Bradburn, were arrested on Wednesday morning after police arrived at the semi-detached house where they lived and they yesterday appeared in court charged with his manslaughter.
Wolverhampton City Council spokeswoman Mel Ryan said today: "Wolverhampton Safeguarding Children Board confirmed this morning that a serious case review would be conducted."
The Wolverhampton Safeguarding Children Board, made up of a number of agencies, can launch a serious case review in a number of instances including when abuse or neglect is known or suspected and either a child dies or is seriously harmed and there are concerns about how organisations or professionals worked together to safeguard him.
Daniel's parents were remanded in custody at Wolverhampton Magistrates Court yesterday afternoon.
Bradburn, aged 33, and Jones, 29, both of Windsor Avenue, will appear at Wolverhampton Crown Court on March 7.
