The crash, on the A449 near Cookley, saw a stretch of the busy Wolverhampton Road closed for two hours.
The elderly passenger of a Silver Vauxhall Astra went to Dudley's Russells Hall Hospital and a female driver in her 30s went to hospital.
The mother, driving a silver Honda Civic, was taken by air ambulance to New Cross Hospital, Wolverhampton suffering from a chest injury and suspected fractured ankle, after the accident just before 2pm yesterday near the Cookley Arms pub, Caunsall.
Her two sons, one aged four, also went to New Cross suffering from shock. One complained of shoulder injuries.
Police closed the road, diverting traffic via Churchill. Fire crews from Stourbridge and Kinver called along with three ambulances and an incident support officer.
