A health and safety investigation has been launched following the tragedy.

The accident happened at S & B Waste Management and Recycling Services in Vicarage Road West, Woodsetton, near Dudley.

It is unclear how the man suffered the crush injuries but he was confirmed dead at the scene.

Police taped off part of the site containing a lorry, a forklift truck and a mound of tyres after the worker, who has not been identified, was injured yesterday.

Four officers from the Health and Safety Executive were called to the site and spoke to staff to gather witness statements and check machinery within the cordon.

Some stunned workers from S & B gathered outside and could be seen talking on mobile phones but none wanted to comment on the tragedy.

Floral tributes have been laid at the entrance to the yard.

A spokesman for the Health and Safety Executive said: "The Heath and Safety Executive is aware of the situation and has visited the site.

"We are investigating alongside the police."
